Prevent Re-Hospitalizations :
Actively monitor condition changes to adjust care plans early and minimize hospital visits. Collaborate on

Communicate directly with physicians regarding medication needs and ongoing care adjustments.
Empower Families :
Teach and counsel patients and family members on managing injuries, diseases, and recovery processes.
Digital Documentation:
Complete patient care records efficiently in our WellSky EMR system.

Requirements:
What We're Looking For Active Michigan LPN License (in good standing) Current CPR Certification Reliable Transportation & current auto insurance
Tech Comfort:
Solid computer skills and ability to navigate an EMR system (WellSky experience is a plus!)
Industry Knowledge :
Working understanding of Medicare/Medicaid guidelines for
