Job Description

Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. In this vital role, you will provide high-quality nursing care to diverse patient populations across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will support patient recovery, comfort, and well-being through comprehensive assessments, medication administration, and personalized care planning. The ideal candidate will demonstrate strong clinical skills, excellent communication abilities, and a commitment to delivering exceptional patient-centered care. 12 Hour Night Shift Duties Perform thorough patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Administer medications and injections accurately following prescribed protocols, including IV infusions and catheterizations. Monitor patients' conditions closely, recognizing signs of acute pain or deterioration requiring immediate intervention. Assist with diagnostic evaluations such as specimen collection, laboratory procedures, and medical imaging preparation. Manage airway devices such as ventilators and perform emergency procedures like first aid or CPR when necessary. Document all patient interactions accurately in electronic health records (EHR) systems while maintaining strict compliance with HIPAA confidentiality policies. Collaborate with physicians, specialists, and interdisciplinary teams to coordinate comprehensive care for patients with complex needs such as dementia or infectious diseases. Support patients in activities of daily living (ADLs), including tube feeding, mobility assistance with Hoyer lifts, and catheter care. Educate patients and their families about health conditions, medication regimens, and self-care strategies. Ensure infection control protocols are rigorously followed to maintain a safe environment for patients and staff. Requirements Valid licensure as a Licensed Practical Nurse (LPN) in the applicable state or region. Minimum of one year of practical nursing experience in settings such as long-term care facilities, hospitals, or outpatient clinics. Experience working with diverse populations including geriatric patients, individuals with developmental disabilities, or those requiring dementia or memory care. Proficiency in using electronic health record (EHR) systems such as Epic, Cerner, Athenahealth, or eClinicalWorks. Knowledge of medical terminology, physiology principles, ICD coding (ICD-9/10), and long-term care regulations. Demonstrated skills in medication administration, IV therapy (including infusion), phlebotomy, and specimen processing. Ability to perform physical examinations and utilize aseptic techniques effectively. Strong communication skills for caregiver interaction and patient education across diverse cultural backgrounds. Commitment to maintaining clinical confidentiality policies while providing compassionate care in fast-paced environments such as emergency rooms or ICU settings. Additional experience in specialized areas such as hospice care, trauma centers (Level I/II), pediatric units (PICU), or surgical units is highly desirable.
