Licensed Practical Nurse (LPN) - Saginaw, Michigan Join Avista Nursing and Rehab's team of dedicated healthcare professionals in Saginaw, Michigan! As a Licensed Practical Nurse (LPN) at Avista Nursing and Rehab, you will play a vital role in delivering high-quality, patient-centered care to our residents. Our state-of-the-art long-term care unit is designed to provide a warm and welcoming environment, and our team of dedicated nurses is available 24/7 to ensure that every resident receives the attention and compassion they deserve. About the
Role:
Provide direct patient care to residents, including administering medications, wound care, and implementing treatment plans. Conduct routine vital sign checks, report changes in condition, and collaborate with the interdisciplinary team to develop and update care plans. Develop and maintain positive relationships with residents, families, and colleagues to ensure a smooth transition from hospital to home. Participate in quality improvement initiatives to improve patient outcomes and satisfaction.
Responsibilities:
Provide skilled nursing care to residents with complex medical and psychosocial needs. Collaborate with other disciplines to develop and implement individualized care plans. Conduct condition-specific education and training for residents and families. Participate in complex wound care, ostomy care, and other special care programs. Maintain accurate and detailed records of care provided and resident outcomes.
