Job Description
Seasons Home Health Care is a clinician-owned home care agency committed to delivering high quality, compassionate care in the comfort of the patient's own home. Founded by healthcare professionals, we understand the importance of clinical excellence and a supportive work environment. We are seeking a Full-Time LPN for St. Joseph County and the surrounding areas and who has the heart and skills to work with home bound patients. Provide care wherever the patient calls home. Carry out the plan of care as directed by the patient's physician. Work with the physician for medication needs and changes to the plan of care. Ability to complete documentation within an EMR System. (WellSky) Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed. When appropriate, teach and counsel family members about care and process for disease/injuries. Attend orientation, meetings and CEUs, Requirements Computer skills; must be able to work within EMR system. LPN's must have current LPN MI license in good standing. Reliable auto and current auto insurance. Current CPR. Can work well independently Solid understanding of Medicare/Medicaid guidelines as it applies to Home Health Full medical insurance benefits, paid time off for full time employees.
