• Minneapolis Prestigious Care Homes is seeking a dependable and detail-oriented Licensed Practical Nurse (LPN) to support medication management, clinical documentation, and compliance oversight across 2 settings (independent living and assisted living).

This role is ideal for a nurse who enjoys a mix of hands-on support, medication oversight, and clinical coordination, and who is comfortable guiding caregivers while maintaining strong documentation standards. Key Responsibilities Medication & Clinical Oversight Set up and review medications for assisted living residents Audit MAR/TAR documentation for accuracy and compliance Coordinate pharmacy refills and physician orders Monitor residents for changes in condition and report appropriately Check medication carts and ensure safe medication practices Documentation & Assessments Assist with admission and ongoing resident assessments Support quarterly reviews and care plan updates Assist with post-hospital return paperwork Help maintain accurate clinical documentation Staff Support & Coordination Serve as a resource to caregivers and med aides Answer clinical questions and assist with problem-solving on the floor Communicate with physicians, pharmacies, and care teams as needed Assist with incident report follow-up and compliance tasks One weekend a month on call Schedule Part-time, approximately 24-30 hours per week Full

• time 32-40 hours per week Flexible scheduling Qualifications Current Minnesota LPN license in good standing Experience in assisted living, long-term care, or home care preferred Strong medication knowledge and documentation skills Ability to work independently across multiple sites Organized, dependable, and team-oriented Compensation $30
