Overview:

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) and/or Registered Nurse (RN) to join our home care nursing team. Our home care nursing program is designed to provide nursing level care for individuals (pediatric to adults) to remain in the comfort of their own homes rather than a facility. Home care nursing shifts range from 4-12 hr shifts; days and nights. We work hard to keep schedules flexible and work around your current schedule needs. As a nurse, you will play a vital role in providing high-quality patient care and ensuring the overall well-being of our patients. Duties:

• Administer medications and treatments as prescribed by physicians
• Monitor patient vital signs and report any significant changes
• Assist with patient assessments and provide appropriate nursing interventions
• Perform basic life support measures in emergency situations
• Access and maintain intravenous (IV) lines as needed, RN only
• Provide patient/family education on various health topics
• Practices nursing with respect for individual, cultural, and spiritual differences
• Maintain accurate and up-to-date medical documentation
• Communicates effectively and in accordance with agency requirements
• Supervises applicable home care team members as delegated by

RN Case Manager Requirements:

• Valid LPN or RN license in Minnesota
• Proficiency in medical terminology and documentation
• Strong knowledge of anatomy and physiology
• Ability to assess patient needs and develop individualized care plans
• Excellent communication and interpersonal skills
• Basic life support (BLS) certification
