Hospice LPN - Weekend On-Call | St. Louis Area Breeze Hospice is Hiring a Weekend On-Call LPN in the St. Louis Area! Position Summary Breeze Hospice is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide skilled nursing care and support to hospice patients and their families during weekend hours. The Weekend On-Call LPN will provide patient care in accordance with the established hospice plan of care and within the LPN scope of practice. This position works collaboratively with the RN, physicians, and other members of the interdisciplinary team to help ensure patients receive timely, compassionate care throughout the weekend. Schedule Full-Time Friday Evening - Monday Morning Weekend on-call coverage In-field visits to patients in private residences, assisted living facilities, skilled nursing facilities, and other care settings as needed Key Responsibilities Provide skilled nursing visits to hospice patients in accordance with the established plan of care and within the LPN scope of practice. Monitor patients for changes in condition, symptoms, comfort, and response to treatment. Collect and document clinical information and promptly communicate significant changes in patient condition to the supervising RN and/or appropriate clinical team member. Administer medications and treatments as ordered and permitted within the LPN scope of practice. Provide wound care and other skilled nursing interventions as ordered. Reinforce patient and caregiver education regarding medications, symptom management, comfort measures, and the established plan of care. Respond to weekend patient and caregiver needs and make in-person visits when clinically indicated. Provide emotional support to patients, families, and caregivers during end-of-life care. Collaborate with RNs, physicians, social workers, chaplains, hospice aides, and other members of the interdisciplinary team. Maintain accurate, complete, and timely clinical documentation for all patient encounters. Follow established hospice policies, procedures, physician orders, and plans of care. Notify the appropriate RN or clinical leadership when a patient's condition requires additional assessment, changes to the plan of care, or interventions outside the LPN scope of practice. Qualifications Current and unrestricted LPN license in the State of Missouri . Minimum of one year of nursing experience preferred. Hospice, palliative care, home health, or related experience preferred. Strong clinical observation and patient-care skills. Ability to recognize and promptly communicate changes in patient condition. Knowledge of symptom management and comfort-focused care preferred. Strong communication, documentation, and interpersonal skills. Compassionate and professional approach to patients and families experiencing end-of-life care. Ability to work independently in the field while maintaining appropriate communication with the RN and interdisciplinary team. Reliable transportation and a valid driver's license. Ability to travel throughout the assigned service area.
