Job Description

Mercy is seeking a dedicated Primary Care Assistant LPN to join our day-shift primary care team. In this role, you'll provide direct patient care, take vitals, administer medications, perform point-of-care testing, and support providers with exams and treatments. You'll coordinate patient flow, document accurately in the EHR, and educate patients on care plans and preventive health. Working in a collaborative, compassionate environment, you'll partner with nurses, physicians, and support staff to deliver safe, high-quality, patient-centered care and contribute to Mercy's mission of improving community health. Responsibilities Provide direct patient care including vitals, screenings, and basic treatments Administer medications and injections per provider orders and protocols Assist providers with exams, procedures, and care coordination Perform point-of-care tests and collect lab specimens accurately Document all care and patient interactions in the EHREducate patients and families on care plans and preventive health Maintain infection control and safety standards in all clinical areas Support patient flow, rooming, and scheduling coordination Communicate effectively with providers, nurses, and support staff Participate in quality improvement and continuous learning activities Required Skills Patient assessment Vital signs measurement Medication administration Phlebotomy and specimen collection Wound care and dressing changes Electronic Health Records (EHR) documentation Infection control procedures Immunization administration Basic life support (BLS) Care coordination
