Duties Administer medications and injections accurately, including IV infusions, while adhering to prescribed protocols and safety standards. Perform comprehensive patient assessments, monitor vital signs, and document clinical observations using electronic health record (EHR) systems such as Epic or Cerner. Assist with physical examinations, wound care, catheterization, and other clinical procedures following aseptic techniques. Provide specialized care for patients with complex conditions such as Alzheimer's disease, dementia, or those requiring memory care. Support patients with acute pain management, palliative care, hospice services, and long-term care needs in compliance with regulatory standards. Collaborate with multidisciplinary teams to develop and implement individualized care plans for patients in settings including skilled nursing facilities, assisted living communities, hospitals, or home care environments. Ensure compliance with HIPAA regulations and maintain strict confidentiality of patient information at all times. Educate patients and their families on health management strategies, medication administration, and post-discharge care instructions. Qualifications Valid licensure as a Licensed Practical Nurse (LPN) in the relevant state or jurisdiction. Proven experience working in medical-surgical units, long-term care facilities, hospitals (including ICU or trauma centers), or outpatient clinics. Knowledge of EMR/EHR systems such as Epic, Cerner, or Athenahealth; proficiency in medical documentation and ICD coding (ICD-9/10). Strong clinical skills including vital signs assessment, venipuncture, phlebotomy, catheter care, and basic life support (BLS/CPR). Experience caring for geriatric patients with complex needs such as dementia or Alzheimer's disease; familiarity with memory care techniques is preferred. Ability to work effectively in high-pressure environments such as emergency rooms or Level I trauma centers; experience in triage is advantageous. Excellent communication skills for caregiver interaction and patient education; ability to work collaboratively within multidisciplinary teams. Knowledge of infection control practices and long-term care regulations; experience working with individuals with disabilities or developmental delays is a plus.
