Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using Electronic Health Record (EHR). Administer medications and injections accurately following prescribed protocols while maintaining strict adherence to HIPAA and clinical confidentiality policies. Assist with medical procedures including catheterization, IV infusion, tube feeding, wound suturing, and sterile processing techniques. Monitor patients' conditions closely, manage acute pain, and provide emergency interventions or first aid when necessary. Collaborate with interdisciplinary teams to coordinate patient care plans and discharge planning. Document all patient interactions thoroughly in medical records, ensuring compliance with
ICD-9/ICD-10
coding standards for accurate billing and reporting. Support infection control practices and maintain a sterile environment during procedures like venipuncture or specimen collection/processing. Skills Proven experience working in healthcare environments. Strong knowledge of medical terminology, physiology, anatomy, and diagnostic evaluation techniques. Expertise in managing complex cases involving dementia care, Alzheimer's disease, geriatrics, or behavioral health needs. Ability to perform physical examinations accurately and conduct triage assessments efficiently for urgent or emergent situations. Excellent communication skills for caregiver interaction and client education on health coaching or chronic disease management strategies.
