Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care in long term care setting. Your expertise will help ensure patients receive personalized, safe, and effective nursing services. Responsibilities Deliver comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Administer medications and injections accurately, including IV infusions and catheter care, following strict protocols and safety standards. Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures. Manage patient care activities such as wound dressing changes, tube feeding, and pain control strategies. Document all patient interactions meticulously using Electronic Health Record (EHR) systems like Point Click Care. Collaborate with interdisciplinary teams to develop discharge planning, case management strategies, and long-term care plans tailored to individual needs. Provide education and support to patients and their families on health conditions such as dementia, Alzheimer's disease, infectious diseases, and post-acute care requirements. Skills Proven experience working with diverse patient populations including adults, geriatrics, or individuals with developmental disabilities. Strong knowledge of medical terminology, anatomy, physiology, and nursing best practices across acute care settings such as ICU or emergency rooms. Skilled in medical documentation using EMR/EHR systems like Point Click Care. Ability to perform clinical procedures including venipuncture and sterile processing techniques. Excellent communication skills for caregiver interaction, patient education, and team collaboration in fast-paced environments like trauma centers or urgent care clinics. Knowledge of infection control protocols and long-term care regulations to ensure safe practice standards. Certification in CPR, first aid, and other relevant credentials required for immediate patient safety response.
