Overview:

We are seeking a compassionate and skilled Home Care Nurse to join our team. As a Home Care Nurse, you will provide personalized skilled nursing care to members in the comfort of their own homes. This is a rewarding opportunity to make a positive impact on the lives of individuals in need. Duties & Skills Conduct nursing assessments and develop individualized plans of care based on client needs. Monitor, identify, and document changes in clients' physical, emotional, and overall health. Administer medications and treatments as ordered and within scope of practice. Maintain accurate, timely, and confidential clinical documentation. Educate clients and caregivers on health conditions, medications, treatments, and disease management. Identify health and safety concerns within the home environment and respond appropriately. Collaborate with healthcare providers, caregivers, and other members of the care team to support coordinated care. Demonstrate strong clinical judgment, communication, organization, and time-management skills. Work independently in a home care setting while following professional nursing standards, organizational policies, and standards of care.

Requirements:
