Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) specializing in long-term care to join our healthcare team. The ideal candidate will possess a strong foundation in patient care, with experience working in skilled nursing facilities, assisted living communities, or similar settings. LPN LONG TERM CARE
Shift:

Hours Nights Shift Details:
6.30 PM to 7.00 AM 36 hours/week. (3
  • 12) 13-week contract Administer medications, including injections and IV infusions, following prescribed protocols and ensuring accurate documentation.
Conduct comprehensive patient assessments, monitor vital signs, and evaluate care needs regularly. Assist with activities of daily living such as mobility support, hygiene, and nutrition management tailored to individual resident needs. Collaborate with interdisciplinary teams to develop and implement personalized care plans that address medical and psychosocial requirements. Maintain detailed medical records using Electronic Health Record (EHR) systems while ensuring compliance with HIPAA confidentiality policies. Provide specialized care for residents with dementia, Alzheimer's disease, or memory impairments, including behavioral health management. Respond promptly to urgent situations or emergencies, utilizing first aid skills and critical care knowledge to stabilize residents until further medical assistance arrives. Skills Proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth for accurate documentation and patient record management. Experience working with geriatric patients in long-term care or skilled nursing facilities; familiarity with hospice and palliative care practices. Knowledge of infection control procedures and aseptic techniques to prevent healthcare-associated infections. Strong understanding of anatomy, physiology, and pharmacology relevant to long-term and acute care settings. Skilled in medication administration including injections, catheterization, tube feeding, and venipuncture procedures. Ability to communicate effectively with residents with diverse needs, including those with developmental disabilities or behavioral health challenges. Familiarity with ICD coding (ICD-9/ICD-10) for accurate medical documentation and billing processes.
