Key responsibilities include conducting wellness assessments, monitoring health changes, managing medications, and performing MAR/TAR audits. The role involves communicating with families, physicians, and care teams, as well as tracking incidents, falls, and follow-up care to promote resident safety. Maintaining accurate documentation and ensuring compliance with regulations are essential, alongside supporting infection control and quality initiatives. Additionally, the nurse trains and supports care team members to uphold high standards of resident care. Qualifications & Skills Active LPN/LVN license Over one year of experience in senior living or clinical settings Strong assessment, documentation, and medication management skills Knowledge of clinical compliance and regulatory standards Excellent communication and problem-solving abilities Key Duties Conduct wellness assessments and monitor resident health Support medication management and compliance audits Communicate with care teams, families, and physicians Maintain documentation and ensure regulatory adherence Support infection control and quality improvement efforts Train and mentor care team members
