$1,700 - $1,768/Weekly Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to patient care, excellent clinical skills, and the ability to work effectively within diverse healthcare settings. As an LPN, you will provide essential nursing services, support patient assessments, administer medications, and collaborate with multidisciplinary teams to ensure high-quality care. Responsibilities Perform comprehensive patient assessments, including vital signs and physical examinations, to inform individualized care plans. Administer medications and injections accurately, including IV infusions and catheter care, following established protocols and safety standards. Assist with medical procedures such as wound dressing changes, tube feedings, phlebotomy, and suturing as required. Maintain detailed and accurate medical documentation within electronic health record (EHR) systems while ensuring compliance with HIPAA regulations. Monitor patients' conditions continuously, recognizing signs of acute pain or deterioration, and escalate concerns appropriately. Support patient mobility and comfort through proper use of assistive devices like Hoyer lifts and ensuring safe transfer techniques. Collaborate with physicians, case managers, and caregivers to coordinate discharge planning, long-term care strategies, and client communication. Uphold infection control practices and maintain sterile techniques during clinical procedures in accordance with healthcare standards. Qualifications Valid licensure as a Licensed Practical Nurse (LPN) in the relevant state or region. Proven experience working in diverse healthcare settings such as hospitals, nursing homes, assisted living facilities, or home care environments. Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth for documentation and patient records management. Knowledge of medical terminology, physiology, ICD coding (ICD-9/ICD-10), and basic life support (BLS) certification. Experience in specialized areas including geriatrics, dementia care (Alzheimer's and memory care), hospice care, or behavioral health is highly desirable. Ability to perform complex clinical tasks such as IV insertion, catheterization, specimen collection/processing, and ventilator management when applicable. Strong communication skills for effective caregiver communication and patient education across diverse populations including pediatric to geriatric patients.
