Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) specializing in Long Term Care to join our healthcare team. The ideal candidate will possess a strong foundation in patient care, with extensive experience in geriatrics, dementia, and memory care settings. As an LPN in Long Term Care, you will provide essential nursing services to residents, ensuring their safety, comfort, and well-being through comprehensive assessments and personalized care plans. Your role will be vital in maintaining high standards of quality care within a supportive environment that promotes dignity and respect for all residents. Duties Perform thorough patient assessments, including vital signs, physical examinations, and health histories to inform individualized care strategies. Administer medications and injections accurately following prescribed protocols while monitoring for adverse reactions. Manage IV infusions and catheter care with strict adherence to aseptic techniques and safety standards. Document all patient interactions, treatments, and observations clearly within electronic health record (EHR) systems in compliance with HIPAA regulations. Collaborate with interdisciplinary teams to develop and implement comprehensive care plans tailored to residents' needs, including dementia and Alzheimer's care. Provide wound care, including dressing changes and pressure ulcer management, ensuring optimal healing conditions. Assist with activities of daily living such as mobility support, feeding assistance, and hygiene to promote resident comfort. Participate in ongoing training related to long-term care regulations, infection control policies, and emergency procedures to maintain certification standards. Skills Proven experience working within long-term care facilities or nursing homes with a focus on geriatrics or memory care. Strong knowledge of EMR/EHR systems such as Epic or Cerner for accurate documentation and patient management. Expertise in medication administration, IV infusion therapy, catheterization, and basic life support (BLS). Familiarity with
ICD-9/ICD-10
coding systems for accurate medical record keeping and billing processes. Ability to perform comprehensive patient assessments while demonstrating excellent caregiver communication skills. Experience working with individuals with developmental disabilities or behavioral health challenges is advantageous. Knowledge of infection control practices, hospice care principles, and acute pain management strategies. Skilled in triage procedures to prioritize urgent resident needs effectively within a fast-paced environment.
