PM Job Summary:
At Elizabeth Park Rehabilitation and Nursing LLC, a leading provider of residential, long-term, rehabilitation, and subacute care, we are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team. As a LPN, you will play a vital role in providing high-quality care to our patients and residents. Responsibilities:
Provide direct care to patients and residents, including administering medications, wound care, and other treatments as prescribed by physicians Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams Monitor and report patient progress, identifying areas for improvement and making recommendations for modifications to care plans Collaborate with other healthcare professionals, including RNs, PTs, OTs, and social workers, to ensure comprehensive care Participate in patient and family education, promoting effective communication and empowerment Maintain accurate and detailed records, adhering to regulatory and facility standards
Requirements:
Current LPN licensure in the state of New Jersey High school diploma or equivalent required; ADN or BSN degree preferred 1+ year of experience as an LPN in a long-term care or rehabilitation setting; experience in rehabilitation and nursing highly valued Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams Ability to lift, transfer, and reposition patients, as well as perform other physical demands of the role commitment to delivering exceptional patient care, with a focus on compassion, respect, and empathy
