Overview WellCare Support Services, a care agency operating under the New Jersey Division of Developmental Disabilities (DDD), is seeking the services of a Licensed Practical Nurse (LPN) to provide nursing support for individuals with developmental disabilities in the Newark, New Jersey area. The ideal candidate will possess a current New Jersey LPN license, demonstrate strong clinical and communication skills, and have a commitment to delivering high-quality, person-centered care. Experience working with individuals with developmental disabilities is preferred. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following physician orders and safety protocols, including IV infusions and medication administration techniques. Assist with diagnostic evaluations, specimen collection and processing, and basic medical imaging procedures to support clinical decision-making. Manage patient care plans by coordinating with multidisciplinary teams, including case management for complex cases involving dementia, Alzheimer's disease, or memory care. Provide specialized care such as wound dressing changes, catheterization, tube feeding, ventilator management, and sterile technique procedures. Support patients in acute pain management, hospice care, or post-acute settings while adhering to HIPAA regulations and maintaining meticulous medical documentation. Educate patients and families on health coaching topics like medication adherence, nutrition, discharge planning, and chronic disease management. Ensure infection control standards are maintained through aseptic techniques and proper sterilization practices in all clinical activities. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice in your jurisdiction. Proven experience working in diverse healthcare environments such as hospitals (including ICU or Level I Trauma Center), nursing homes, assisted living facilities, or outpatient clinics. Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/ICD-10), CPT coding, and electronic medical records systems like Epic or Athenahealth. Experience with specialized areas including critical care (ICU), emergency medicine (ER), geriatrics, pediatrics, or behavioral health is highly desirable. Ability to perform technical procedures such as phlebotomy, venipuncture, catheter care, suturing assistance, and airway management. Excellent patient assessment skills combined with compassionate communication tailored to individuals with disabilities or cognitive impairments like dementia or Alzheimer's disease. Physical ability to stand for extended periods and perform lifting or repositioning tasks using equipment such as Hoyer lifts when necessary.
