Job Description

Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will help ensure patients receive safe, effective, and personalized nursing services. This position offers an exciting opportunity to make a meaningful difference in the lives of diverse patient populations, including geriatric individuals, those with developmental disabilities, and patients requiring specialized care such as Alzheimer's or memory care. You will work closely with interdisciplinary teams to support patient recovery, comfort, and well-being through skilled nursing interventions. Duties Perform comprehensive patient assessments by monitoring vital signs, evaluating physical conditions, and documenting findings accurately in Electronic Health Records (EHR) systems such as Epic or Cerner. Administer medications and injections safely following HIPAA guidelines and medical documentation standards. Provide specialized care for patients with complex needs including IV infusion therapy, catheterization, wound care, tube feeding, and ventilator management. Assist with diagnostic evaluations such as specimen collection, processing, and basic laboratory procedures. Support acute pain management strategies and coordinate with physicians for effective treatment plans. Educate patients and their families on health management techniques including medication administration, nutrition, and disease-specific care like dementia or hospice needs. Collaborate with healthcare teams in various settings—hospital units like ICU or trauma centers, skilled nursing facilities, or outpatient clinics—to ensure continuity of care and discharge planning. Maintain strict adherence to safety protocols including infection control practices and sterile techniques during procedures such as suturing or sterile processing. Use EMR/EHR systems proficiently to document patient care activities accurately while ensuring confidentiality in compliance with HIPAA regulations. Participate in case management activities to optimize patient outcomes through effective communication and coordinated care plans. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice in your jurisdiction. Proven experience working in diverse healthcare environments such as hospitals (including ICU or emergency room), long-term care facilities, assisted living communities, or home health settings. Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks; experience with medical coding systems ICD-9/10 and CPT coding is preferred. Strong knowledge of physiology, anatomy, pharmacology, and medical terminology relevant to patient assessment and treatment. Experience working with geriatric patients or individuals with disabilities including dementia or Alzheimer's disease is highly desirable. Skilled in IV insertion, medication administration (including injections), venipuncture, catheter care, first aid procedures, and airway management techniques such as ventilator support. Ability to perform physical examinations accurately while maintaining clinical confidentiality policies at all times. Excellent communication skills for caregiver interaction and client education; ability to work effectively within multidisciplinary teams. Critical care experience such as ICU or trauma center exposure is a plus; familiarity with acute pain management and infectious disease care is advantageous. $28.00 - $33.00 per hour Expected hours: 20.0 - 40.0 per week
