Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN) - Staff Nurse, where your expertise will directly impact patient care and well-being. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems. Administer medications, injections, IV infusions, and other treatments following strict protocols while ensuring patient safety and comfort. Manage complex care needs such as wound care, catheterization, tube feeding, ventilator management, and pain control. Collaborate with multidisciplinary teams to develop and implement individualized care plans tailored to each patient's needs. Conduct triage assessments for urgent and non-urgent cases, prioritizing patient needs efficiently in fast-paced environments like emergency rooms or urgent care clinics. Document all patient interactions accurately in medical records while maintaining strict adherence to HIPAA and clinical confidentiality policies. Assist with diagnostic evaluations, specimen collection/processing, and medical imaging procedures as required. Provide education to patients and families on health management strategies including dementia care, Alzheimer's care, hospice support, and chronic disease management. Support discharge planning and case management efforts to ensure seamless transitions from hospital or long-term care facilities to home or community settings. Experience Valid licensure as a Licensed Practical Nurse (LPN) with current state certification. Demonstrated experience working with diverse populations including geriatrics, pediatrics, neonatal patients, or individuals with disabilities. Prior clinical experience in settings such as hospitals (including ICU or trauma centers), skilled nursing facilities, assisted living communities, or home health environments. Proficiency in EMR/EHR systems like Epic or Cerner for accurate documentation and patient record management. Experience with specialized areas such as critical care (ICU), emergency medicine (ER), long-term care regulations, hospice care, infectious disease management, or medical-surgical units is highly desirable. Knowledge of

ICD-9/10

coding standards for accurate medical documentation and billing processes. Skills in advanced procedures including IV insertion/infusion therapy, sterile technique, venipuncture, catheter care, and basic life support (BLS). Familiarity with medication administration protocols and pain management strategies for acute and chronic conditions.
