Weekly direct deposit AMX Healthcare is seeking an experienced and dependable Licensed Practical Nurse (LPN) for a full-time home care opportunity in Manhattan, New York. This position is ideal for an LPN who is comfortable working independently in the community and providing skilled nursing services to patients in their homes. Candidates should have strong clinical judgment, excellent documentation skills, and experience caring for patients with chronic and medically complex conditions. Schedule Monday through Friday 9:00 AM - 5:00 PM Full-time, consistent weekday schedule Responsibilities Provide skilled nursing care to patients in their homes according to established plans of care. Perform ongoing patient observations and identify changes in condition. Administer prescribed medications, injections, and treatments. Assist with medication management and patient education. Perform dressing changes and basic wound care as ordered. Provide G-tube and enteral-feeding care when required. Support patients managing chronic conditions such as diabetes, CHF, COPD, hypertension, and other ongoing medical conditions. Assist patients transitioning home following hospitalization, rehabilitation, surgery, or illness. Monitor patients for complications and communicate significant findings to the supervising RN, physician, or appropriate clinical team. Educate patients and families regarding medications, disease management, safety, and the established plan of care. Maintain accurate and timely clinical documentation. Communicate effectively with physicians, nurses, therapists, case managers, patients, and families. Travel between assigned patient homes throughout the Manhattan service area. Experience We Are Looking For Candidates should have a strong background in hands-on clinical nursing care , preferably within home health, community nursing, skilled nursing, rehabilitation, long-term care, or a similar setting. Experience with the following is especially valuable: Home health or visiting-nurse care. Caring for adult and geriatric patients in their homes. Medication administration and medication reconciliation. Diabetes, CHF, COPD, hypertension, and chronic-disease management. Wound care and dressing changes. Injections and other ordered nursing treatments. G-tube or enteral-feeding care. Monitoring patients following hospital or rehabilitation discharge. Recognizing and reporting changes in patient condition. Patient and family education. Working independently while communicating closely with an interdisciplinary clinical team. Completing detailed nursing documentation in a timely manner. Previous home care experience is strongly preferred. Candidates should be comfortable traveling within the community and managing assigned patients with limited direct supervision. Requirements Active New York State LPN License in good standing. Current CPR/BLS certification. Previous LPN clinical experience required. Home care, community health, rehabilitation, skilled nursing, or long-term care experience strongly preferred. Strong medication-administration and patient-assessment skills. Current physical and tuberculosis screening. Required immun
