Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems. Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards. Assist with medical procedures such as catheterization, tube feeding, wound suturing, and sterile dressing changes while maintaining aseptic techniques. Monitor patients' conditions closely, identify changes in health status, and communicate findings promptly to the healthcare team for appropriate intervention. Support triage activities by evaluating patient symptoms and determining urgency levels in urgent care or emergency settings. Collaborate with multidisciplinary teams to develop and implement personalized care plans for patients with complex needs such as dementia, Alzheimer's disease, or developmental disabilities. Ensure compliance with HIPAA regulations and maintain strict confidentiality of all medical records and documentation. Educate patients and their families on health management strategies, medication administration, and post-discharge care instructions. Experience Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Proven experience working in diverse healthcare environments such as hospitals, long-term care facilities, skilled nursing facilities, assisted living communities, or home care settings. Demonstrated proficiency in EMR/EHR systems like Epic or Cerner for accurate documentation and patient data management. Hands-on experience with critical care procedures including ventilator management, ICU protocols, trauma response (Level I & II Trauma Center experience), or emergency medicine is highly valued. Familiarity with specialized areas such as hospice care, infectious disease management, geriatrics, memory care (including Alzheimer's and dementia), pediatric nursing including neonatal ventilator use or PICU experience. Knowledge of ICD coding (ICD-9/10), CPT coding for procedures, medication administration techniques including IV insertion/insertion care and phlebotomy. Ability to perform physical examinations, vital signs assessment, specimen collection/processing, and basic life support (BLS/CPR).
