  • $37 per hour We are currently seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team in Vestal, NY in providing exceptional care to our residents in a long-term care (LTC) setting.

Responsibilities:
Primary Care:
Provide direct nursing care to residents in accordance with their individual care plans. This includes administering medications, performing routine treatments, and monitoring vital signs.
Assessment and Documentation:
Conduct comprehensive assessments of residents' health and review their medical histories. Accurately document and communicate any changes in residents' conditions to the Registered Nurse (RN) or healthcare providers.
Collaboration:
Collaborate with interdisciplinary team members, including physicians, RNs, certified nursing assistants (CNAs), and therapists, in developing and implementing care plans. Ensure effective communication and continuity of care.
Supportive Care:
Offer emotional support and ensure residents' comfort and well-being. Assist with activities of daily living, such as bathing, dressing, and toileting, to maintain their dignity and promote independence.
Education:
Provide health-related education and support to residents and their families. Offer guidance on disease management, medication administration, and any necessary lifestyle modifications.
Safety and Compliance:
Adhere to infection control policies, safety guidelines, and nursing standards of practice. Maintain accurate and up-to-date documentation, using electronic medical record systems as required by the facility.
Quality Improvement:
Participate in quality improvement initiatives and adhere to established protocols and procedures. Actively contribute to creating a positive and nurturing environment for residents What You'll Need to Succeed Active NY LPN License . Current BLS Certification .
