Responsibilities Perform comprehensive patient assessments, including vital signs and physical examinations, to evaluate health status and identify urgent needs. Administer medications, injections, IV infusions, and other treatments following strict protocols and safety standards. Utilize EMR (Electronic Medical Record) systems such as Epic or Cerner to document patient care accurately and efficiently. Assist with complex procedures such as catheterization, tube feeding, wound suturing, and sterile dressing changes using aseptic techniques. Collaborate with multidisciplinary teams to develop and implement personalized care plans, including discharge planning and case management. Conduct triage assessments for urgent care or emergency situations, prioritizing patient needs effectively. Support patients with specialized needs such as Alzheimer's care, dementia care, memory care, or working with individuals with developmental disabilities. Maintain infection control standards and ensure compliance with HIPAA regulations to protect patient confidentiality. Provide education on health coaching topics like nutrition, medication management, pain control, and post-acute care instructions. Monitor patients on ventilators or with acute pain management requirements in inpatient or outpatient settings. Qualifications Valid LPN license in good standing; previous experience in critical care, hospital medicine, or emergency room settings is highly preferred. Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth for accurate medical documentation. Experience working with diverse patient populations including geriatrics, pediatrics, neonatal ventilator care, or individuals with disabilities. Knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), and diagnostic evaluation techniques. Skilled in performing venipuncture, phlebotomy, injection administration, catheter care and sterile processing procedures. Ability to handle complex clinical situations involving acute pain management or infectious disease care confidently. Strong communication skills for effective patient assessment and collaboration within multidisciplinary teams. Certification in Basic Life Support (BLS) required; additional certifications such as IV insertion or hospice care are advantageous.
