Pinnacle Care Providers is expanding, and we're looking for passionate professionals to join our team as Care Coordinators ! As a Care Coordinator, you'll work side-by-side with our Physicians and Nurse Practitioners in skilled nursing, long-term care, assisted living, and independent living facilities. Medical and Life Insurance Continuing Education Fund What You'll Do:
Be the bridge between providers, facilities, patients, and families. Coordinate patient care and organize census data. Pre-round with providers, gathering and presenting key medical information. Scribe patient encounters and ensure accurate EMR documentation. Track visits and progress notes to maintain consistent care. Bill patient visits accurately and on time.
