Additionally, this position will be responsible for ensuring all medical record information is protected, complete, and accurate. Follows direction of the QAPI Coordinator to assist with process improvement activities and reporting internally as well as regulatory agencies. Competitive Pay. Pay negotiable, based on experience.
Responsibilities QAPI Assistant:
Identify and communicate documentation deficiencies to QAPI coordinator to improve documentation for accurate risk adjustment. Collaborate with hospice team members to implement measurable, actionable solutions that will result in improved accuracy for documentation. Supports QAPI Coordinator on charting regulations and changes as it relates to quality and risk adjustment to ensure compliance. Performs analysis and provides feedback to hospice team members on a regularly scheduled basis. Assist with infection control. Supports the development and implementation of quality improvement interventions under the direction of the QAPI Coordinator. Audits and assists in resolving deficiencies impacting QAPI plan compliance. Monitors and investigates quality of care and collaborates with IDT to determine next steps for action. Collects and summarizes performance data and identifies opportunities for improvement. Maintain participant flags in the EMR. Update and monitor patient code status and advance directives in the EMR Participates in site visit preparation and execution by regulatory agencies (State, CMS). Provide training for new clinical staff members, assist to set up filters and permissions within the software system. Assist in monitoring and tracking orders. Requirements Current state license as a Licensed Practical Nurse 1 year of hospice or MDS experience, preferred Exceptional written and oral communication skills Proficient with Microsoft Office Strong computer skills Valid Driver's license and reliable transportation Participate in on-call as assigned
