In this vital role, you will provide compassionate, skilled nursing services across various settings, including long-term care facilities, assisted living communities, hospitals, and home health environments. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using electronic health record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following physician orders, including IV infusions and medication management for pain control or chronic conditions. Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures when necessary. Support patients with activities of daily living (ADLs), including tube feeding, catheter care, wound dressing changes, and mobility assistance utilizing equipment like Hoyer lifts. Monitor patients' responses to treatments, manage acute pain episodes, and escalate concerns promptly to physicians or advanced practice providers. Maintain strict adherence to HIPAA regulations and clinical confidentiality policies while documenting all patient interactions thoroughly in EMR/EHR systems like Athenahealth or eClinicalWorks. Collaborate with multidisciplinary teams to develop discharge plans, coordinate case management efforts, and ensure compliance with long-term care regulations. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice in the relevant healthcare setting. Proven experience working in diverse environments such as long-term care facilities, hospitals (including ICU or trauma centers), hospice care, or outpatient clinics.
