Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using electronic health record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following physician orders, including IV infusions and medication management for pain control or chronic conditions. Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures when necessary. Support patients with activities of daily living (ADLs), including tube feeding, catheter care, wound dressing changes, and mobility assistance utilizing equipment like Hoyer lifts. Monitor patients' responses to treatments, manage acute pain episodes, and escalate concerns promptly to physicians or advanced practice providers. Maintain strict adherence to HIPAA regulations and clinical confidentiality policies while documenting all patient interactions thoroughly in EMR/EHR systems like Athenahealth or eClinicalWorks. Collaborate with multidisciplinary teams to develop discharge plans, coordinate case management efforts, and ensure compliance with long-term care regulations. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice in the relevant healthcare setting. Proven experience working in diverse environments such as long-term care facilities, hospitals (including ICU or trauma centers), hospice care, or outpatient clinics. Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, or similar platforms; experience with

ICD-9/ICD-10

coding is preferred. Strong knowledge of physiology, anatomy, infection control protocols, and medical terminology essential for accurate patient assessment and documentation. Experience working with geriatric populations or individuals with developmental disabilities; familiarity with dementia care (including Alzheimer's and memory care) is highly valued. Ability to perform clinical procedures such as venipuncture, phlebotomy, sterile technique for wound care or suturing, airway management, and first aid. Excellent communication skills for caregiver interaction and patient education; ability to work effectively within a multidisciplinary team. Physical ability to stand for extended periods, lift patients safely using assistive devices like Hoyer lifts, and perform routine clinical tasks efficiently.
