We are seeking a compassionate and clinically skilled Licensed Nurse (RN or LPN) to join our specialized respiratory and tracheostomy unit. Patient Assessment & Monitoring Perform focused and comprehensive nursing assessments. Obtain and monitor vital signs, including blood pressure, pulse, respirations, oxygen saturation, temperature, and blood glucose levels, identifying and reporting significant changes. Observe residents and document pertinent conditions, responses, and clinical observations. Recognize changes in condition and communicate findings to physicians, advanced practice providers, supervisors, and other appropriate team members in accordance with facility policies. Medication Administration Administer medications and treatments as ordered by authorized providers and in accordance with nursing standards, facility policies, and applicable regulations. Administer oral, topical, subcutaneous, enteral, and intravenous medications within the scope of licensure and competency. Follow safe medication administration practices, including controlled substance accountability and required documentation. Treatments & Specialized Procedures Provide direct bedside nursing care for residents with complex medical needs. Provide wound care, including management of negative pressure wound therapy (wound VACs), as appropriate. Insert and maintain urinary catheters in accordance with physician orders and facility protocols. Collect laboratory specimens, including urine, stool, and sputum samples. Perform injections, enemas, EKGs, and point-of-care testing, as permitted by licensure and competency validation. Follow established infection prevention and isolation protocols. Perform tracheostomy care, oral and tracheal suctioning, oxygen therapy management, and enteral feeding support, including PEG and NG tube care, consistent with physician orders, facility policies, and demonstrated competencies. Documentation & Communication Maintain accurate, timely, and complete clinical documentation. Communicate significant changes in condition, treatment responses, and care needs to residents, families, physicians, and the interdisciplinary team, as appropriate. Participate in care planning activities and assist with updating resident care plans, ADL documentation, and related clinical records based on resident needs and changes in condition.
Required Qualifications:
Current, active, and unencumbered Oklahoma Licensed Practical Nurse (LPN) or Registered Nurse (RN) license. Minimum of one (1) year of experience in LTAC, step-down ICU, tracheostomy care, or another high-acuity clinical setting. Minimum of one (1) year of nursing experience in a skilled nursing, long-term care, post-acute, or related healthcare setting.
Preferred Qualifications:
Experience caring for residents requiring respiratory support, tracheostomy management, enteral nutrition, or other complex clinical interventions. Demonstrated ability to thrive in a fast-paced, interdisciplinary environment while delivering compassionate, resident-centered care.
