Duties Provide comprehensive nursing care to patients in various settings, including medication administration, wound care, and vital sign monitoring Assist with patient assessments, physical examinations, and documentation using Electronic Health Record (EHR) systems such as Point Click Care Administer IV infusions, injections, and perform catheterization with precision and adherence to sterile techniques Support patients with dementia or Alzheimer's disease through specialized memory care practices and behavioral health strategies Collaborate with interdisciplinary teams on discharge planning, case management, and diagnostic evaluations to optimize patient outcomes Manage acute pain and airway issues, including ventilator management and airway maintenance when necessary Ensure compliance with HIPAA regulations and maintain strict confidentiality of all medical records and documentation Experience Valid LPN license with current state licensure required; prior experience working in long-term care facilities or skilled nursing facilities highly preferred Demonstrated proficiency in EMR/EHR systems such as Epic, Athenahealth, or eClinicalWorks for accurate medical documentation Critical care experience in hospital settings such as ICU or Level I Trauma Center environments is highly desirable Experience working with individuals with disabilities or developmental delays enhances your ability to provide personalized patient care Knowledge of ICD coding (ICD-9/ICD-10), CPT coding, and medical terminology supports accurate recordkeeping and billing processes Prior exposure to hospice care, infectious disease management, or emergency medicine adds valuable expertise to your skill set Strong communication skills for effective caregiver communication and patient education in diverse healthcare settings Join us in delivering exceptional patient-centered care that transforms lives!
