Shift:

Monday-Friday 8AM-5PM |

Full-Time Qualifications:

Required:

• LPN license state of Oregon
• Minimum of 1 year in Home Health or Med/Surg
• Current BLS/CPR.
• Valid drivers license and insured vehicle

Preferred:

• 1 year of Home Health experience
• Experience administer medications and treatments as prescribed
• Experience performing wound care, catheter care, and other clinical procedures
• Strong assessment, care coordination, and patient education skills

Job Summary:

• Deliver individualized nursing services to patients within the home setting in accordance with established plans of care.
• Conduct ongoing patient assessments, identify changes in clinical status, and communicate concerns to the appropriate healthcare provider.
• Carry out physician-ordered medication administration, therapeutic interventions, and other prescribed treatments.
• Provide hands-on clinical care, including wound management, catheter maintenance, and additional nursing procedures within scope of practice.
• Guide patients and caregivers on medications, treatment plans, symptom management, and strategies for safely managing care at home.
