Responsibilities:

Provide direct patient care as part of a multidisciplinary team, including administering medications, performing vital signs, and conducting treatments Develop and implement care plans tailored to individual patients' needs, taking into account their physical, emotional, and social well-being Collaborate with other healthcare professionals to ensure seamless communication and coordination of patient care Participate in shift bed counts, patient flow, and other administrative tasks as needed Demonstrate a commitment to patient-centered care and a strong understanding of the importance of compassion, empathy, and respect in the delivery of patient care

Requirements:

Current LPN licensure in the State of Pennsylvania High school diploma or equivalent required Previous experience in a long-term care or subacute setting preferred
