Nursing Home / Long-Term Care Job Summary AMX Healthcare is seeking a compassionate, dependable, and dedicated Licensed Practical Nurse (LPN) for a full-time position supporting residents in nursing home and long-term care settings in the Chicora/Worthington, Pennsylvania area. This hands-on nursing role focuses on providing quality resident care, administering medications and treatments, monitoring residents' conditions, and working closely with RNs, CNAs, providers, and the care team to meet each resident's individual needs. The position is well suited for an LPN who enjoys building meaningful relationships with residents while providing safe, compassionate, and dependable care throughout each shift. Key Responsibilities Deliver nursing care according to individualized care plans, physician/provider orders, and facility policies while promoting resident comfort and dignity. Safely administer medications and prescribed treatments, monitor residents' responses, and maintain accurate medication and treatment documentation. Observe residents for changes in physical, cognitive, or emotional condition and promptly report significant changes to the RN, provider, or appropriate nursing leadership. Complete required nursing assessments and obtain vital signs and other clinical information as appropriate for the resident and facility. Work closely with RNs, CNAs, providers, and other members of the interdisciplinary care team to ensure residents' needs are addressed. Document assessments, medications, treatments, interventions, and resident responses accurately and in a timely manner. Follow infection-control procedures, fall-prevention practices, medication-safety protocols, and all facility policies and procedures. Provide respectful, compassionate care while promoting residents' privacy, independence, comfort, and overall well-being. Qualifications Active LPN license valid for practice in Pennsylvania. Current BLS/CPR certification preferred/required based on facility requirements. Previous experience in a nursing home, skilled nursing, or long-term care setting preferred. Strong clinical, communication, documentation, teamwork, and time-management skills. Compassionate and professional approach to resident care. Ability to work the required full-time schedule, including every other weekend.
