LPN Job Duties May Include:
Provide direct patient care under the supervision of a Registered Nurse, in accordance with the Nurse Practice Act and home health care policies Performs selected prescribed treatment and nursing measures for the patient under specific guidelines. Administer prescribed medications and treatments per physician/RN orders Prepare accurate and timely clinical notes and nursing documentation Monitor and report changes in patient condition to the RN Care Plan Coordinator Reinforce patient/family education as directed by the RN or physician Report any changes in the patient's condition to the office staff. Communicate effectively with RNs, office staff, patients, families, and other members of the home health care team Maintain patient privacy and confidentiality (HIPAA) Respond appropriately to emergencies and initiate CPR/emergency procedures when necessary Works under the direction and supervision of the RN Case Manager / Clinical Supervisor (DON). May provide direction to and reinforce the plan of care with home health aides and ancillary personnel within the scope of the LPN role; does not independently supervise or evaluate staff unless designated and permitted by agency policy and state regulation. Maintain current nursing skills and clinical competence. Ask questions when in doubt for clarification by the Care Plan RN Coordinator. Perform other nursing duties within the LPN scope of practice as assigned
Requirements:
Graduate of an accredited practical nursing program. Valid, active Pennsylvania LPN license (required) 1 year of nursing experience preferred; however, new grads and less-experienced LPNs are welcome to apply Valid driver's license, state ID or passport Current CPR / Basic Life Support (BLS) certification (American Heart Association) (cost will be covered by the agency if course is completed in-house) Current physical exam with PPD/chest x-ray results Successful completion of criminal and child abuse background clearances Ability to lift a minimum of 50 lbs.and reposition bed-bound clients Reliable transportation (must be able to travel to patients) Ability to read, write and communicate effectively in English Ability to work independently in the home setting, exercise sound clinical judgment, and remain calm during emergencies. Ability to organize, prioritize, and manage time effectively during shift. Compassion, patience, and cultural sensitivity in working with patients and families.
