Responsibilities:

Provide direct patient care to a diverse range of residents, including those with complex medical conditions and rehabilitation needs Assist with medication administration, vital signs, and other clinical tasks as needed Develop and implement individualized care plans in collaboration with the interdisciplinary team Document patient information accurately and timely in accordance with regulatory requirements Participate in quality improvement initiatives to enhance patient outcomes and satisfaction Demonstrate a commitment to safety, dignity, and respect for all residents and colleagues About the

Requirements:
