Join our dedicated team in South Mountain, PA, a picturesque location offering a serene environment to practice compassionate and skilled nursing care. Make a meaningful impact on the lives of Veteran residents in a long-term care setting. Responsibilities Provide practical nursing care and treatment for Veteran residents with medical, physical, cognitive, and behavioral health needs. Implement individualized nursing care plans and perform direct resident care in coordination with the interdisciplinary team. Prepare, administer, and accurately document medications and treatments prescribed by physicians. Observe, assess, and report changes in residents' physical, cognitive, and emotional conditions. Take and record vital signs, collect specimens, and maintain accurate clinical documentation and progress notes. Participate in Veteran-centered rehabilitation, restorative, and habilitation programs, including assistance with activities of daily living and therapeutic activities. May serve in a charge role for a unit or care area, supervising unlicensed personnel and supporting continuity of care. Perform nursing duties in accordance with federal/state regulations, facility policies, Veterans' care standards, and LPN scope of practice. Collaborate with RNs, physicians, and interdisciplinary team members to support resident care planning and outcomes. Respond to changes in resident condition and escalate clinical concerns to appropriate licensed personnel. Maintain compliance with infection control, safety protocols, and quality care standards. Participate in care planning meetings, interdisciplinary reviews, and ongoing resident evaluations. Requirements Current Licensed Practical Nurse (LPN) license issued by the Pennsylvania State Board of Nursing, or a valid non-renewable temporary practice permit. 2 years of experience preferred. AHA BLS certification. #ITLTC
