Peaceful Hearts Hospice | Elkhorn, NE Peaceful Hearts Hospice is looking for a compassionate and dependable Licensed Practical Nurse (LPN) to join our team and provide meaningful, high-quality end-of-life care to our patients and their families. This is a primarily field-based position , providing care in patients' homes, skilled nursing facilities, assisted living communities, and other care settings. You'll work closely with an RN Case Manager and our interdisciplinary team to ensure each patient receives individualized, compassionate care. Dental & vision coverage Health insurance reimbursement Mileage reimbursement Short-term & long-term disability Group life insurance 401(k) with employer matching About the Role As an LPN at Peaceful Hearts Hospice, you'll provide direct nursing care to hospice patients under the supervision of an RN Case Manager. You'll help carry out individualized plans of care, manage symptoms, provide treatments, educate patients and families, and communicate closely with the hospice interdisciplinary team. The LPN reports to the Director of Patient Care Services (DPCS) and works under the clinical supervision of the RN Case Manager (RNCM) . What You'll Do Follow individualized plans of care developed by the hospice team. Provide treatments and nursing procedures as ordered, including wound care, symptom management, tube care, bowel regimens, and other appropriate interventions. Monitor patients for changes in condition and communicate concerns promptly to the RNCM and physician. Provide pain and symptom management and evaluate patients' responses to treatment and medications. Educate patients, families, and caregivers about disease processes, medications, symptoms, and what to expect throughout the end-of-life process. Collaborate with RNs, hospice aides, social workers, chaplains, bereavement staff, and other members of the interdisciplinary team. Ensure patients have necessary medications and supplies. Maintain accurate and timely documentation in our electronic medical record system. Provide care in a variety of settings, including private homes, skilled nursing facilities, assisted living communities, and other residential settings. Provide emotional support and guidance to families and caregivers. Participate in team meetings, quality improvement activities, and community hospice education. Maintain professional licensure, continuing education, and compliance with hospice and infection-control requirements.
What We're Looking For Required:
Current, unrestricted Nebraska LPN license in good standing. Graduate of an accredited Practical Nursing program. Valid driver's license. Reliable transportation. Current automobile liability insurance.
Preferred:
At least 1 year of LPN experience . Experience in hospice, home health, oncology, or a similar setting . Hospice experience is preferred, but we're also interested in LPNs who have a genuine passion for providing compassionate care and are interested in learning the hospice philosophy. What It's Like Working With Us Hospice nursing is meaningful work, but it can also be challenging. We are looking for someone who can remain calm and compassionate in emotionally difficult situations, communicate effectively, work independently, and be a dependable member of a team. Work Environment This is a field-based position . Most patient care takes place in the community, with time in the office for meetings, phone calls, and other administrative responsibilities. Depending on job duties and agency needs, some charting and administrative work may be completed remotely.
Pay:
$29.00 - $33.00 per hour Expected hours: 40.0 per week
