Job Summary We are seeking a dedicated and energetic Telephone Triage Nurse / Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will serve as the frontline communicator, providing expert nursing advice, patient assessment, and guidance over the phone. Your expertise will help determine urgent needs, coordinate appropriate care pathways, and ensure patients receive timely, compassionate support. Responsibilities Conduct thorough patient assessments via telephone, utilizing clinical judgment to identify urgent versus non-urgent needs Provide clear instructions for symptom management, medication administration, and follow-up care based on established protocols Document all patient interactions accurately within Electronic Health Record (EHR) systems, ensuring compliance with HIPAA and confidentiality policies Collaborate with the healthcare team Triage calls related to a wide range of medical concerns including acute pain, infectious diseases, dementia care, and chronic conditions such as diabetes or hypertension Maintain up-to-date knowledge of medical terminology, ICD coding (International Classification of Diseases), and current clinical guidelines Skills Proven experience working with Electronic Medical Records (EMR) systems Knowledge of triage protocols and procedures for urgent and emergent situations Familiarity with diverse patient populations including geriatrics and pediatrics Skilled in medical documentation, vital signs assessment, medication administration including injections and basic life support (BLS) certification Ability to communicate effectively with patients of all ages while demonstrating empathy and professionalism Understanding of HIPAA regulations and clinical confidentiality policies to ensure patient privacy at all times Join our team where your expertise will directly impact patient well-being!
