Duties The Primary LPN/Project Coordinator leads clinical operations for the mobile health unit, serving as the sole on-site clinical provider. This role is responsible for managing daily operations, conducting patient assessments and screenings, providing direct nursing care and care coordination, and delivering mobile health services. Clinical responsibilities include chronic disease screenings, wound care, and facilitating telehealth visits with remote providers to ensure high-quality, patient-centered care. Qualifications Must be a Licensed Practical Nurse and hold an Associate's Degree in Nursing (ADN), with 2-3 years of clinical nursing experience. Valid South Carolina Driver's License and ability to provide proof of a clear driving record. Ability to lift and transport equipment and supplies and perform the physical demands associated with mobile clinical operations.
