Overview Join our dedicated healthcare team as a 3rd Shift Floor LPN/RN in a Long-Term Care (LTC) or Skilled Nursing Facility (SNF), working from 11 pm to 7 am. Responsibilities Deliver comprehensive patient assessments, including vital signs, physical examinations, and medication administration, ensuring accurate documentation within Electronic Health Record (EHR) systems. Administer IV infusions, injections, catheter care, and manage airway and ventilator support as needed for residents with complex medical needs. Monitor residents' conditions closely, recognizing signs of acute pain or infection, and respond promptly with appropriate interventions. Collaborate with interdisciplinary teams to develop and implement individualized care plans that align with long-term care regulations and best practices. Maintain strict adherence to HIPAA and clinical confidentiality policies while documenting medical records and medical documentation accurately. Conduct triage assessments for emergent issues during overnight hours, coordinating with physicians or emergency services when necessary. Support residents with dementia or Alzheimer's disease through specialized memory care techniques and compassionate communication. Experience Current licensure as an LPN or RN with active state licensure in good standing. Prior experience working in long-term care facilities, skilled nursing facilities, or assisted living environments is highly preferred. Demonstrated proficiency in EMR/EHR systems such as Epic, Cerner, or Athenahealth; experience with electronic documentation is essential. Knowledge of

coding, hospice care protocols, infection control procedures, and acute care practices including ICU or trauma center experience is advantageous. Strong clinical assessment skills with familiarity in managing complex cases involving infectious diseases, pain management, tube feeding, catheterization, and ventilator support. Experience working with geriatric patients and individuals with disabilities or developmental challenges enhances your ability to deliver personalized care during overnight shifts.
