Job Description
Overview McCoy Memnorial Nursing Home Join our dedicated team as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) on the nursing home floor, providing compassionate, high-quality long-term care to our residents. You will be instrumental in delivering personalized patient care, ensuring safety, and supporting overall well-being within a dynamic team committed to excellence. Duties Administer medications, including injections and IV infusions, following established protocols and ensuring precise documentation in electronic health records (EHR/EHR systems). Conduct comprehensive patient assessments by monitoring vital signs, evaluating physical conditions, and performing routine physical examinations to inform care plans. Collaborate with interdisciplinary teams to develop, implement, and update individualized care plans tailored to each resident's needs, including dementia, Alzheimer's, memory care, and hospice support. Assist with activities of daily living (ADLs), such as catheter care, tube feeding, wound dressing, and mobility assistance using Hoyer lifts or other equipment. Manage acute pain and infectious disease cases by applying specialized knowledge in pain management and infection control protocols. Document all patient interactions accurately while maintaining strict adherence to HIPAA regulations and clinical confidentiality policies. Provide education and support to residents and their families about health conditions, medication management, and post-discharge care planning. Requirements Valid licensure as an LPN or RN with current state registration or licensing. Proven experience working in long-term care facilities or skilled nursing environments; experience with geriatrics strongly preferred. Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth for accurate medical documentation. Knowledge of long-term care regulations, ICD coding (ICD-9/ICD-10), and best practices in dementia and Alzheimer's care. Ability to perform complex patient assessments including vital signs, physical examinations, and diagnostic evaluation procedures. Strong communication skills for caregiver interaction, client communication, and team collaboration. Competence in infection control practices, sterile processing techniques, catheterization, phlebotomy, and basic life support (CPR). Critical thinking skills with experience managing acute care situations such as ICU or emergency room scenarios; trauma center experience is a plus. Ability to work effectively with individuals with disabilities or developmental challenges; experience in assisted living or home & community care is advantageous.
