In this role, you will provide high-quality patient care in a supportive environment, ensuring the well-being of our residents and patients. Your expertise will contribute to the overall health and comfort of those in your care, particularly in settings such as assisted living, long-term care, and memory care facilities. Responsibilities Administer medications and treatments as prescribed by physicians. Conduct patient assessments and monitor vital signs. Assist with daily living activities and personal care for residents. Collaborate with healthcare teams to develop and implement patient care plans. Maintain accurate medical documentation and records in compliance with HIPAA regulations. Provide education and support to patients and their families regarding health management. Participate in triage and emergency response as needed. Ensure a safe and clean environment for patients. Skills Candidates should possess a variety of skills that enhance their ability to provide exceptional care, including but not limited to: Proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks. Experience in critical care, long-term care, assisted living, or hospice settings. Knowledge of medical terminology, ICD coding (ICD-9 & ICD-10), and medication administration. Skills in IV insertion, phlebotomy, catheterization, tube feeding, and airway management. Familiarity with geriatrics, dementia care, Alzheimer's care, and working with individuals with developmental disabilities. Ability to perform physical examinations and diagnostic evaluations. Experience in acute pain management, emergency medicine, or hospital medicine is highly valued. Strong communication skills for effective patient interaction and teamwork.
