Responsibilities Deliver comprehensive nursing care to residents, including medication administration, vital signs monitoring, and physical assessments. Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each resident's needs. Manage complex clinical procedures such as IV infusions, catheterizations, wound care, and tube feeding with precision and adherence to safety protocols. Conduct triage assessments for incoming residents or visitors, prioritizing urgent needs and coordinating appropriate interventions. Document all patient interactions accurately using electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA and confidentiality policies. Monitor residents' conditions closely, recognizing signs of acute pain or infection and escalating concerns promptly. Educate residents and their families about health management strategies, including dementia care, Alzheimer's support, and medication regimens. Requirements Valid LPN or RN licensure in good standing with the state licensing board. Proven experience working in long-term care facilities or skilled nursing settings; experience with geriatrics highly preferred. Strong knowledge of medical terminology, anatomy, physiology, and long-term care regulations. Hands-on experience with EMR/EHR systems such as Epic, Cerner, or Athenahealth for accurate documentation. Competency in clinical procedures including IV insertion, venipuncture, sterile technique, first aid, and basic life support (BLS). Ability to perform comprehensive patient assessments and vital signs monitoring efficiently. Excellent communication skills for caregiver collaboration and client interaction; sensitivity when working with individuals with disabilities or dementia.
