Overview We are seeking a dedicated and experienced Licensed Practical Nurse (LPN) specializing in long-term care to join our dynamic healthcare team. Manage complex cases involving Alzheimer's care, dementia, memory care, and behavioral health needs with sensitivity and professionalism. Provide wound care, tube feeding management, and perform sterile procedures including catheterization and suturing. Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and post-acute care services. Maintain accurate medical documentation within Electronic Health Record (EHR) systems such as Epic or Cerner while ensuring compliance with HIPAA regulations. Assist with diagnostic evaluations like specimen collection/processing and support infection control practices. Educate patients and caregivers on medication administration, health coaching, and disease management strategies. Respond effectively to urgent situations including acute pain management, airway management, or emergency interventions in high-acuity environments such as Level I Trauma Centers or ICU settings. Ensure adherence to long-term care regulations and safety standards across all clinical activities. Skills Valid licensure as a Licensed Practical Nurse (LPN) with current state certification. Extensive experience working in long-term care facilities such as nursing homes or skilled nursing facilities. Proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth; familiarity with

ICD-9/ICD-10

coding is preferred. Critical care experience including ICU or emergency medicine environments is highly desirable. Knowledge of geriatrics, dementia care (including Alzheimer's), hospice care, and behavioral health principles. Skilled in IV infusion therapy, medication administration, venipuncture, phlebotomy, and aseptic techniques. Strong understanding of physiology knowledge related to patient assessment and diagnostic evaluation. Excellent communication skills for caregiver communication and client education across diverse populations. Ability to adapt quickly to different clinical settings while maintaining high standards of patient safety and confidentiality. Familiarity with medical terminology relevant to internal medicine, pediatrics (including neonatal ventilator management), trauma centers, or surgical units enhances versatility.
