Position Summary We are seeking a skilled and detail-oriented Wound Care Nurse (LPN) to assess, treat, and manage acute and chronic wounds for residents. The Wound Care Nurse collaborates with the nursing and medical team to develop effective wound care treatment plans and promote healing. Responsibilities Assess, stage, and document all wounds, including pressure injuries, surgical wounds, and skin tears. Develop and implement individualized wound care treatment plans in collaboration with physicians and the interdisciplinary care team. Provide education and training to nursing staff on wound prevention, assessment, and treatment best practices. Monitor wound healing progress and adjust treatment plans as needed. Ensure wound care documentation complies with state and federal regulations and facility policies. Perform other duties as assigned by facility management. Qualifications Active, unencumbered Licensed Practical Nurse (LPN) license in the state of Tennessee. Wound care certification (WCC, CWOCN, or similar) preferred. Long-term care/skilled nursing experience preferred. Strong communication and interpersonal skills and a genuine interest in working with older adults.
