Overview We are seeking a compassionate and highly skilled Hospice Nurse to join our dedicated healthcare team. The ideal candidate will provide exceptional end-of-life care, ensuring comfort and dignity for patients with terminal illnesses. This role requires a strong foundation in various nursing specialties, including palliative medicine, critical care, and long-term care, with proficiency in electronic health records and medical documentation. The Hospice Nurse will collaborate closely with families, physicians, and interdisciplinary teams to deliver personalized patient-centered care in diverse settings such as home care, inpatient facilities, and assisted living communities. Duties RN Conduct comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine care needs. Administer medications and treatments such as IV infusions, injections, tube feedings, and wound care using aseptic techniques. Manage complex symptom control including acute pain management, dyspnea relief, and comfort measures aligned with hospice standards. Coordinate discharge planning and facilitate transitions between inpatient and home-based hospice services. Utilize EMR/EHR systems like Epic, Cerner, Athenahealth, or eClinicalWorks for accurate medical documentation and record keeping. Educate patients and families on disease processes, medication administration, advanced directives, and end-of-life planning. Collaborate with interdisciplinary teams to develop individualized care plans that incorporate memory care, dementia or Alzheimer's care strategies. Ensure infection control protocols are strictly followed to prevent healthcare-associated infections. Provide emotional support and health coaching to patients with complex needs including those with developmental disabilities or behavioral health challenges. Maintain compliance with HIPAA regulations while managing sensitive patient information. Skills RN Proven experience in hospice & palliative medicine or related fields such as geriatrics, oncology, or chronic disease management. Proficiency in using EMR/EHR systems such as Epic, Cerner, or Athenahealth for documentation and patient tracking. Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and diagnostic evaluation techniques. Skilled in medication administration including injections, IV insertion/infusion therapy, catheterization (urinary or vascular), ostomy care, and sterile processing. Ability to perform physical examinations and assessments accurately; experience with vital signs monitoring is essential. Familiarity with specialized procedures such as spinal taps or gastrostomy tube management is preferred. Excellent communication skills for working with patients of all ages—including children—and their families; experience with pediatric or neonatal ventilator management is a plus. Knowledge of infection control practices and aseptic techniques; experience in hospital medicine or critical care environments such as ICU or Level I Trauma Centers is advantageous. Ability to work independently in home & community settings while maintaining high standards of patient safety and comfort.
