DUTIES AND RESPONSIBILITIES 1.

Will obtain RN Visit Frequency (calendar) for patients and enter plotted visits into computer program for each individual patient. 2. Will enter visit types on patient's frequency as ordered by MD to include (i.e., UA, Lab Draws, B12, Insulin, etc.) and coordinate with QA to schedule all labs for each patient and ensure they are performed. 3. Produces a daily schedule by 3:00 p.m. and presents to Director of Clinical Services. Who will review the schedule for review and approval. 4. Produces a weekly schedule every Friday at 4 p.m. for the upcoming week and 6. Will schedule notes/reports needed for RN (i.e., transfer, discharge summaries, etc.). 5. Will reschedule missed visits within the same week if a patient is relocated with prior approval of RN Clinical Manager and/or designee. 6. Will provide RN's with upcoming recertification's (3 week prior) for weekly clinical meetings. 7. Will coordinate with health care team and clients of needed changes in schedule that may come up on a daily basis. 8. Build professional relationship(s) with clients to ensure excellent service delivery. 9. Show respect for patients/clients by maintaining confidentiality 10. Will assist in medical records department with uploading/scanning of nursing/therapy documentation for patient's electronic clinical record. 11. Will communicate professionally with patients, staff and/or MD offices regarding agency processes or procedures. 12. Any other assigned duties as deemed necessary by Immediate Supervisor.
