Job Overview We are seeking a dedicated Licensed Practical Nurse (LPN) to join our compassionate healthcare team. The ideal candidate will possess strong clinical skills, a passion for elder care, and experience working with individuals with memory impairments. As an LPN, you will play a vital role in delivering high-quality nursing care, supporting residents' health and well-being while ensuring compliance with regulatory standards. Duties Provide direct patient care, including medication administration, minor first aid and vital signs monitoring. Assist residents with activities of daily living, ensuring comfort and dignity at all times. Collaborate with interdisciplinary teams to develop and implement individualized care plans. Document resident health status accurately using electronic health records (EHR/EHR systems). Monitor residents for signs of acute or chronic health issues, including Alzheimer's and dementia-related conditions. Support infection control protocols and maintain sterile techniques during procedures such as catheterization and phlebotomy. Educate residents and their families on health management, medication use, and safety procedures. Skills Valid licensure as a Licensed Practical Nurse (LPN) in accordance with state regulations. Experience working with geriatric patients, including memory care, Alzheimer's, and dementia populations. Proficiency in electronic health records (EHR) systems such as Epic or Cerner. Knowledge of medication administration protocols and IV infusion techniques. Strong assessment skills with the ability to recognize changes in residents' conditions promptly. Familiarity with long-term care regulations, HIPAA compliance, and infection control policies. Excellent communication skills to effectively collaborate with team members, residents, and families.
