We are seeking a compassionate LVN to join our mobile wound care team and support patients with acute and chronic wound conditions.

Job Summary:

The Licensed Vocational Nurse (LVN) - Mobile Wound Care will provide direct patient care, assist in wound assessment and treatment, and collaborate with providers and facility staff to ensure optimal wound healing. This role requires strong clinical skills, attention to detail, and the ability to work independently while traveling between patient locations.

Key Responsibilities:

Assist in comprehensive wound assessments , including wound staging, measurements, and documentation. Perform wound care treatments , including dressing changes, debridement (as per licensure scope), and negative pressure wound therapy application. Monitor wound healing progress and report changes to the Nurse Practitioner (NP) or Medical Director . Educate patients, caregivers, and facility staff on wound prevention and proper wound care techniques. Maintain accurate and timely documentation in the electronic health record (EHR) system. Follow wound care protocols and physician orders to ensure effective treatment plans. Assist with ordering wound care supplies and ensuring proper inventory for patient needs. Travel to multiple patient locations within the designated service area. Adhere to infection control standards , HIPAA regulations, and company policies.

Qualifications & Skills:

Active LVN license in the state of practice. Minimum of 1-2 years of clinical nursing experience (wound care experience preferred). Certification in Wound Care (WCC, CWON, or CWCN) preferred but not required. Experience in home health, skilled nursing, or long-term care is a plus. Strong understanding of wound care treatments, dressings, and therapies . Ability to work independently and manage a mobile patient caseload . Excellent communication, critical thinking, and organizational skills. Willingness to travel within the designated service area (valid driver's license required).
