Job Description

Aveanna Healthcare is seeking a compassionate In Home Healthcare LVN to deliver high quality nursing care to patients in the comfort of their homes. In this role, you'll provide hands on clinical care, administer medications and treatments, and closely monitor patient conditions. You'll collaborate with families and an interdisciplinary team to implement individualized care plans, educate caregivers, and promote safety and comfort. Responsibilities Provide skilled in home nursing care to pediatric and adult patients per physician orders. Administer medications, treatments, and therapies while monitoring patient responses. Complete accurate, timely clinical documentation and care notes. Collaborate with families, caregivers, and the interdisciplinary team on care plans. Educate patients and families on disease management, equipment, and safety. Monitor and report changes in patient condition to supervising RN/physician. Maintain infection control, patient safety, and compliance with company policies. Operate and troubleshoot home medical equipment (e.g., ventilators, feeding pumps).
