SOUTH AUSTIN, TX
Are you a compassionate Licensed Social Worker with experience in skilled nursing and long-term care? We are seeking an experienced professional to join our interdisciplinary team at a well-established skilled nursing and rehabilitation facility located in South Austin, Texas . This is an excellent opportunity for a social worker who is passionate about supporting residents and families through every stage of the care journey while making a meaningful impact in a post-acute care setting. Relocation assistance and sign-on bonus are available and open for discussion based on experience and candidate needs. Key Responsibilities Support residents and families throughout the admissions, care planning, and discharge processes Conduct comprehensive psychosocial assessments upon admission and as needed Participate in interdisciplinary care plan meetings and advocate for resident needs Provide emotional support, counseling, and guidance to residents and families Assist with advance care planning and end-of-life discussions Coordinate referrals for behavioral health, dental, vision, hearing, and other support services Address resident and family concerns while collaborating with facility leadership Complete required documentation, including social services and MDS-related components Conduct routine resident rounds to identify and address psychosocial needs Participate in weekend Manager-on-Duty rotations as scheduled Support quality assurance and regulatory compliance initiatives Qualifications Bachelor's or Master's Degree in Social Work (BSW or MSW) Current Texas Licensed Social Worker (LSW) license required Previous skilled nursing facility (SNF) experience strongly preferred Long-term care, rehabilitation, or post-acute care experience highly desired Strong assessment, documentation, communication, and problem-solving skills Ability to build trust with residents, families, and interdisciplinary team members Compassionate, resident-centered approach to care Why Consider This Opportunity?
